Lyon County, KY (March 24, 2026) – A serious crash involving two concrete mixer trucks left one man dead on Monday, March 23. The accident happened around 7:20 a.m. on KY Highway 94, just east of the KY Highway 564 intersection.

Authorities said both trucks were traveling in opposite directions when they clipped each other as they met on the road. The impact caused both drivers to lose control, sending the vehicles off the roadway. One truck overturned into a nearby creek, while the other came to rest in a field.

Officials identified the driver who died as 48-year-old Jeremiah A. Smith of Eddyville. He was found inside the overturned truck in the creek and was pronounced dead at the scene. The other driver, 24-year-old Conner A. Paschall of Hazel, suffered minor injuries and declined treatment.

Emergency units responded to the scene and worked to assist those involved. Crews also managed the crash site as part of the response and investigation.

Authorities continue to investigate the cause of the crash.
